Can you use sick days for vacation? It depends on how your time off is counted.
Whether sick days can be used for a vacation, or vacation days for being sick, comes down to one thing: does your company keep one bank of paid time off for everything, or separate banks for vacation and sickness? State paid sick leave laws add rules on top.
Here's how it usually works, what managers should know, and how to keep sick days from quietly turning into vacation, or the other way round. It's general information, not legal advice.
One PTO bank: it's all the same days
Many US companies have a single PTO bank: vacation, sick days and personal days all come from it. In that case the question doesn't really exist. A sick day uses PTO, a vacation day uses PTO, and you decide how to spend them.
The downside: people come to work sick to save days for vacation. Many companies with a PTO bank add a few separate sick days, or treat illness more flexibly, to stop that.
Separate banks: usually not for vacation
When sick leave and vacation are separate, sick leave is usually only for being sick, medical appointments, or caring for a sick family member. Using it for a planned trip is generally against the policy.
Going the other way is usually allowed: if you're sick and out of sick days, most companies let you use vacation days instead.
What state sick leave laws change
A growing number of states and cities, including California, New York, New Jersey, Washington, Arizona and Oregon, require employers to give paid sick leave. These laws generally define what sick leave can be used for (your own illness, a family member's, medical appointments, and in some places safety reasons like domestic violence) and limit what employers can ask for as proof.
Some of these laws also affect carryover and payout. Check the rules where your people work, and ask HR before you write or change a policy.
For managers: plan for sick days, don't police them
You can't plan sick days, but you can plan for them: no single point of failure on critical work, and a backup for on-call. A team where nobody can be sick without something breaking will see people working sick.
Keep it fair and simple
Say what sick days are for
Your own illness, a family member's, appointments. Write it down.
Don't ask for notes for one day
Asking for proof for a single day pushes people to come in sick, and some states limit it.
Let vacation cover sickness
If someone runs out of sick days, let them use vacation without fuss.
Track sick days separately
Even in a PTO bank, seeing sick days helps you spot burnout.
Check your state
Paid sick leave laws set minimums and rules on proof and carryover.
Plan backups
Every critical area and on-call shift should have a second person.

When someone calls in sick, the question is what stops. Here's a team board with who covers what. Add a sick day to anyone and see which areas lose their only person.
Who covers what
Tag what each person can handle and the board names the days nobody covering an area is in.
On-call with backups
On-call shifts sit next to time off, with a warning when they clash.
Sick days as their own type
Sick days are a separate kind of time off, so you can see them apart from vacations.
