Some companies give one bank of PTO for everything: vacation, being sick, personal errands. Others give vacation and sick leave separately. The difference sounds like accounting, but it changes how people behave, especially whether they come to work sick.
Here's what each covers, the trade-offs, and what state laws add. It's general information, not legal advice.
What each one covers
Sick leave is for being ill, injured, getting medical care, and usually caring for a sick family member. Many state laws also cover safety reasons like domestic violence. It's not meant for planned trips.
PTO in a combined bank covers anything: vacation, sickness, appointments, personal days. You decide how to use it, and nobody asks why.
One PTO bank: pros and cons
Pros: simple to run, no one has to explain why they're off, and people who are rarely sick get more vacation. Cons: people come to work sick to save days for a trip, and someone with a long illness can lose their whole vacation to it.
Separate banks: pros and cons
Pros: sick days are protected, so people stay home when they're ill, and vacation is real vacation. Cons: more rules, and some people feel they have to justify a sick day.
Many companies with a PTO bank now add a few separate sick days to get the best of both.
What state sick leave laws change
A growing number of states and cities, including California, New York, New Jersey, Washington, Arizona, Oregon and Massachusetts, require paid sick leave. A combined PTO bank can satisfy those laws if it gives at least as much and can be used the same way, but the rules on notice, proof and carryover differ by place. Check your state labor department before writing a policy.
Make either model work
Never make people prove a single sick day
It pushes people to come in sick, and some states limit it.
Show sick days separately
Even in one bank, tracking sick days helps you spot burnout.
Let vacation cover long illnesses
If someone runs out of sick days, let them use vacation without a fight.
Write it down
What counts as sick leave, how much notice, what happens at year end.
Check your state
Paid sick leave laws set minimums and rules on use and proof.
Plan backups
Every critical area and on-call shift should have a second person.

Questions
What's the difference between PTO and sick leave?
Sick leave is for illness, medical care and caring for sick family. PTO in a combined bank covers anything, including vacation and sickness, with no reason needed.
Is it better to have PTO or separate sick leave?
Separate sick leave protects vacation and keeps sick people at home. A combined PTO bank is simpler and gives healthy people more vacation. Many companies combine a PTO bank with a few separate sick days.
Can PTO be used as sick leave?
In a combined bank, yes. With separate banks, most companies let you use vacation when you've run out of sick days.
Does PTO count toward state paid sick leave laws?
Often, if the PTO policy gives at least as much time and can be used the same way the law requires. Rules differ by state and city.
